Scheduling a New Patient Appointment

If you are interested in being a new patient at Twelve Rivers Medicine, please fill out the new patient inquiry below to be contacted when I have availability. Please be sure to add twelveriversmedicine@gmail.com to your contacts or you may not receive my response.
Please note: I only bill Regence, United Healthcare and Kaiser PPO (not HMO or Core). A superbill will be provided if you would like to try for reimbursement from other insurance companies.
New patient appointments include an intake covering the patient’s health history with special attention paid to the chief concerns. A personalized acupuncture treatment is created for each patient based on their symptoms, patterns and health history. Adjunct techniques like cupping and essential oil therapy may be included. Please fill out the form below if you would like to be a new patient at Twelve Rivers Medicine. Please include gillian@twelveriversmedicine.com in your contacts or my response will possibly go to your spam folder.
The number and frequency of follow-up treatments is based on the severity and history of the condition, which will be explained as part of the initial treatment plan and may include herbs and nutritional recommendations. Allow an hour and a half for your initial visit to Twelve Rivers Medicine (TRM). Follow up treatments are one hour.
After we make you a new patient appointment, please go to the Patient Portal to fill out new patient paperwork at least 24 hours before your appointment. You will also receive a confirmation email with a link to the portal.
Returning patients please check in the portal to see if your contact and insurance information is up to date and upload any lab tests or images you wish to discuss.
I have a firm 48-hour cancellation policy unless you are sick or in an emergency situation.
Twelve Rivers Medicine is accepting new patients paying out of pocket and with the following insurance companies, billed as an in-network provider: Regence, Kaiser Permanente PPO and United Healthcare. Note: I am no longer accepting new patients with Premera, Lifewise, Aetna or Cigna unless you are able to pay out of pocket. I am happy to give you a superbill for reimbursement.
If TRM is not in your insurance network, you may pay at the time of service and we offer visit statements for reimbursement. To find out if your insurance covers treatment at TRM, use this Insurance Verification Form when you call your insurance company before your first treatment. Be sure to fill out your insurance information in the Patient Portal.
